Kvp+\- 8 at low 52KVp calibration mobile pack.
KVp goes directly to service mode on display when shoot 120 kv in KVp cal.
No reading kv on kiethley at times during KVp- no errors.
No KVp on kietthley during KVp cal yet developed film shows x-Ray with good image.
At times 469 when shooting at different kv repeat exposure is same.
Kietley used in another room and works fine.
One possibility is weak batteries. How old are they? Have you done a load test?
Other:
Check if K7 contactor (AMX1A4 panel) pulls in
Inspect contactors K1 - K6 on same panel
Bad crimp on red lead to A of Q3 on same panel
Defective jumper harness from J3 on 1KHz to J7 on Fil/KV board
HV cathode cable shorted
Fil/KV board defective
1 KHz board defective
Spark gaps on HV transformer should be .010"
Check black and white leads on Q3 block (AMX1A4 panel)
Q3 block defective
When you say "KVp goes directly to service mode on display when shoot 120 kv in KVp cal." Do you mean that it kicks you out of the KVP Calibration when you try to preform the 120 KV level calibration?
It is important to set up your meter prior to going into the generator calibration, because the collimator light will not come on after you have started the calibrations. I use the Lamp Time Calibration to get the meter set up optimally with the collimator opened correctly, although i always used a RadCal.
Another important thought, Do you have any sort of "Portable Pack" for the AMX4 on the Kiethly? How its calibration works is much different than a standard R&F room. Your rad meeter needs to measure BACK from the end of the exposure to find the proper KVP. I don’t remember how many ms, but if your meter is triggering on the leading edge of the exposure, your measurements will be way off because of the ramp up time and the kVp pulses.
As for your error 469 -
The X-Ray On status went low during the exposure.
During Calibration;
Does the start command get to the inverter?
Does the safety contractor pull in?
*Is high voltage produced? if so, is the resulting kVp what was selected?
Do the correct tap relays pull in?
Check kV feedback signals. It should look like the signal from the High Voltage Divider
I believe you are getting this problem because of the reason I * The Calibration is not passing correctly, and when making exposures the internal KV sensor tells the CPU to give that error code.
First, I would make sure your meter is setup to work with a portable (contact the company if needed) Then I would check the batteries if they are failing to give enough Amps during your 120 kVp exposures.

469 error is because I can not make the shot calibration. Your problem may be simple.

why not allow you calibration
check the year of manufacture of the batteries.
once the stretch batteries are well begins to perform the calibration from the beginning and slowly. and without skipping any steps.
