Philips pairs chronic condition patients with customized care via new virtual platform

Philips has unveiled a new telemedicine solution connecting patients with experts virtually to customize health plans and provide them with tools to better manage their chronic conditions.

Using connected devices and engagement tools to collect information and facilitate timely interventions, as well as get doctors up to speed to reduce workflow gaps, Philips Virtual Care Management is expected to reduce emergency department visits and cost of care.

The solution uses condition-specific protocols for customizing plans for diabetes, hypertension, heart disease, chronic kidney disease, chronic obstructive pulmonary disease (COPD), as well as gestational programs for diabetes and hypertension.

“Care providers and health systems today are often short on time and resources, accelerating the need to find new ways to gain visibility into patients’ health amid an overwhelming variety of options,” said Nick Wilson, general manager of ambulatory virtual care at Philips.

Chronic condition management makes up 90% of healthcare expenditures in the U.S., according to Wilson.

Studies show that Philips Virtual Care Management solutions and services reduced ED visits by 38% and saved $3,086 in annual claims per patient or member. Patients also saw an average 3.06% reduction in HbA1c (blood glucose) levels, and were discharged faster, with fewer 30- and 90-day hospitalizations required.

Philips showcased Philips Virtual Care Management on March 28 during a session at ViVE 2023 in Nashville.

The solution is not available outside of the U.S.
